The Australian Nurse-Family Partnership Program (AFPP) is a nurse-led home visiting program based on the US Nurse-Family Partnership. The program empowers and informs first-time Indigenous mums or mums whose partner is Aboriginal or Torres Strait Islander and supports them to reach their personal goals and grow strong, healthy families.
The AFPP is a voluntary program designed to support the mother having their first baby from pregnancy up until the child is two years of age, with regular home visits from a Nurse Home Visitor (NHV) and an Aboriginal or Torres Strait Islander Family Partnership Worker (FPW). This relationship-based program incorporates five client centered principles:
· Follow your heart’s desire
· You are the expert in your life
· Focus on strengths
· Focus on solutions
· Only a small change is necessary.
Known locally as the First Time Mums Program, its aim is to improve pregnancy outcomes and child health and development by supporting women to make good health choices, and empowering mothers to be the best parent possible.
The program also helps parents develop a vision for their own future by planning for future pregnancies and/or continuing their education and finding work.
