Wuchopperen is a Community Controlled Aboriginal Health Organisation providing holistic health care services to the Aboriginal and Torres Strait Islander people of Cairns.

1 x Full Time, Temporary 30 June 2022

Based in Cairns

The Transition Care Case Coordinator will provide culturally secure coordinated transition from care including - therapeutic case management, tailored individual supports, crisis intervention and outreach supports to vulnerable young people with significant trauma backgrounds, diverse and complex needs, and often, high risk taking behaviours.

The position would suit an applicant who holds a relevant qualification together with experience in a similar field.

The above is an Identified position. It is a genuine occupational requirement that an Identified position be filled by an Aboriginal person or Torres Strait Islander person, as permitted by and arguable under section 25 of the Queensland Anti-Discrimination Act (1991).
