Outreach Worker (Identified)

Salary $71,130 – $76,274

The Outreach Worker within the Integrated Team Care (ITC) program is responsible for connecting clients to programs and services. Their role is to support Care Coordinators within the ITC program to assist clients to attend the medical and/or specialist appointments.

This includes provision of a comprehensive, culturally appropriate, chronic conditions management service in delivering outreach health care programs as a member of a multi-disciplinary team, delivering services consistent with the Wuchopperen Model of Care guidelines and within defined clinical governance procedures and legislative regulations.
